LCMC Health Patient Portal Proxy Access

As a parent or guardian, you can access your child's account through your own LCMC Health Patient Portal account. Here are two ways to gain access to your child’s account depending whether you have a LCMC Health Patient Portal account:

  1. You have a myLCMC account.
    To gain access to your child's account,

    1. Click here

    2. Login to LCMC Health Patient Portal, and under the Messaging tab, click Request Family Access.

    3. Complete and submit the form.

  2. You do not have a LCMC Health Patient Portal account.
    To gain access to your child's account,

    1. Click here

    2. Complete and submit the form. Once approved, you will receive an email with your activation code and instructions on how to activate your LCMC Health Patient Portal account. If you fill out the Child Information section on the form, access to your child’s account may be granted simultaneously. If not or you want to request access to additional accounts, continue with the following steps.

    3. Login to LCMC Health Patient Portal, and under the Messaging tab, click Request Family Access.

    4. Complete and submit the form.

LCMC Health Patient Portal accounts are granted to patients who are 18 years of age or older.

    Your teen, considered age 13 to 17, must approve the proxy access request privately, in person at his or her next doctor visit.
