The target population is HIV-infected women, their partners, children, youth and their families in five distinct geographic areas in Louisiana. The primary site of service delivery (59%) is through the program in New Orleans (mostly urban and suburban), with 28% of the funds being subcontracted in the Baton Rouge area, and the remaining 13% to providers in southwestern, northwestern and northeastern Louisiana (largely rural areas). Among all sites, 1,183 Part D clients received services in 2006. The majority of clients served by the project were racial/ethnic minorities (New Orleans 90%, Baton Rouge 90%, Shreveport 70%, Lafayette 63% and Monroe 87%). In FY 2007, the project anticipates serving approximately 1,300 Part D eligible clients of similar demographics.

FACES offers a one-stop shop model that enables on-site access to medical case management, mental health and substance abuse counseling and support groups, health education and adherence counseling, child care, transportation assistance, peer advocacy support and related services. Part D medical services in New Orleans are provided at the HIV Outpatient Clinic and at Children’s Hospital. In Baton Rouge, Lafayette, Shreveport and Monroe, Part D funding supplements Part B services and supports medical case management, mental health, peer support, and client transportation. Statewide, Part D supports 31.47 FTE of whom 72% are racial or ethnic minorities.
